In patients with\u00a0known pathogenesis (secondary osteoarthrtis),\u00a0the disease is most commonly due to mechanical bone deformation, termed hip dysplasia. The\u00a0treatment is planned on the basis of clinical,\u00a0anatomic and biomechanical characteristics of\u00a0the hip joint. The biomechanical pattern is most\u00a0frequently determined by the Wiberg centre-edge\u00a0angle of lateral covering of the femoral head.\u00a0Dysplastic hips seem to be characterized by an\u00a0unfavourable stress distribution to the weightbearing area. A mathematical model is used to\u00a0find out which geometric parameters in addition\u00a0to the Wilberg centre-edge may influence the distribution of stress on the weight-bearing surface.\u00a0The author describes the method for assessing\u00a0these parameters from a standard antero-posterior rentgenogram. The procedure was tested\u00a0on healthy subjects with small centre-edge\u00a0angles, reporting no clinical problems.\u00a0We analyzed the mathematical model for determining stresses exerted on the weight-bearing\u00a0articular surface of the hip during the one-legged\u00a0stance, and determined the parameters affecting stress distribution in the hip joint for various\u00a0hip and pelvis configurations. Standard anteroposterior rentgenograms of 411 subjects were\u00a0examined and the subjects with centre-edge\u00a0angles smaller than 30\u00b0 were selected. The\u00a0mathematical model was used to determine the\u00a0values of radial stress for the selected subjects.\u00a0The following parameters are relevant to the\u00a0assessment of hip dysplasia: the radius of the\u00a0sphere that corresponds to the hip weight bearing surface r, the magnitude of the force R, and the parameter termed functional angle of the\u00a0femoral head covering, which is used here for\u00a0the first time and equals the sum of the centreedge angle and the angle of the vertical inclination of the resultant hip force R. Further, it was\u00a0found that subjects with small centre-edge\u00a0angles are relatively rare in the healthy population. In only 9 of the 411 subjects examined\u00a0was this angle smaller than 30\u00b0, and in 4 of them\u00a0it was less than 20\u00b0. In 5 of these 9 subjects the\u00a0peak stress values were found to be comparable with the reference values of <em>in vivo<\/em> mesurements. This is attributable to the compensating\u00a0effects of the favorable hip and pelvis geometry and low body weight.\u00a0The functional angle of the femoral head covering, the femoral head radius and the size of\u00a0the force R are the parameters that can be\u00a0determined from a standard AP roentgenogram\u00a0using a relatively simple method and therefore\u00a0constitute valuable elements for assessing hip\u00a0dysplasia.
