A 33-year-old patient with fever, inguinal lymphadenopathy and ankle ulcers returning from the south of Africa

We present a 33-year-old patient who developed high grade fever after returning from his 3-week journey in the south of Africa. He presented with 3 ulcers involving his left ankle and left-sided painful inguinal lymphadenopathy. Laboratory tests yielded low platelet count and mildly elevated liver aminotransferase levels. Ulcer smear and blood cultures grew no pathogenic bacteria or parasites.
